A culturally enhanced Chronic Kidney Wellness Initiative to provide holistic services for individual’s, families and communities.

A team of providers that includes:

  • Nurse Practitioner
  • Traditional Knowledge Keeper
  • Primary Care Chronic Disease Nurse
  • Dietitian
  • Diabetes Nurse Educator
  • Health Navigator
  • Pharmacist

Services Provided

Multidisciplinary chronic disease care, including but not limited to:

  • Primary care services for conditions such as diabetes, high blood pressure, kidney disease, coronary artery disease, and cholesterol management
  • Individual and family/group health education and counseling
  • Dietitian and Diabetes Educator services
  • Traditional & Cultural Services
  • Tele-health support
  • Visiting specialists
  • Outreach services and health clinics to surrounding communities

Traditional & Cultural Services

  • Traditional medicine
  • Traditional and Spiritual Institutes
  • Spiritual counselling

Clients may be referred by their primary care provider, other health care worker or make their own appointment

Collaboration and referral with other health care professionals to address all aspects of health and well being:

  • Emotional Health
  • Spiritual Health
  • Cultural Health
