Health Navigator (Acute Care) – ANHH/Health Services

The File Hills Qu’Appelle Tribal Council is a non-profit organization that provides a range of programs and services to 11 First Nations throughout Southern Saskatchewan within the Treaty Four Territory. The Health Navigator will work as a key member of the Acute Care team within the All Nations’ Healing Hospital (ANHH) located in Fort Qu’Appelle, SK. This position offers a meaningful opportunity to support patients and families within a culturally responsive, integrated, and fast-paced rural hospital setting.

Reporting to the Director of Acute and Clinical Services, or designate, the Health Navigator serves as a central resource to support patients presenting to the Emergency Department and those admitted to the ANHH who require complex discharge planning and care coordination.

DUTIES & RESPONSIBILITIES:

  • Utilize a holistic model to assess patient and family medical, social, cultural, and spiritual needs.
  • Advocate for patients and families through their care journey with navigation, resource connection, and follow-up case planning.
  • Promote culturally safe and trauma-informed care practices by utilizing cultural and traditional supports including First Nations’ Cultural Practitioners.
  • Lead and coordinate complex discharge planning and family meetings for patients admitted in the hospital, including liaising with community-based services.
  • Address barriers to care, including transportation, access to income supports, housing, and mental health.

QUALIFICATIONS:

  • Recognized health degree (i.e., Social Work, Nursing, M.A. Psychology)
  • Current registration with appropriate professional licensing body
  • Valid Saskatchewan Driver’s License
  • Current CIPC and Vulnerable Sector Check

KNOWLEDGE, SKILLS, & ABILITIES:

  • Strong understanding of programs within Ministry, SHA, ISC/NIHB, and community-based services.
  • Excellent interpersonal communication (oral and written) and crisis management skills.
  • Knowledge of discharge planning, case management, and system navigation.
  • Strong advocacy skills and ability to work with complex and vulnerable populations.
  • Ability to work independently and as a member of a team.
  • Demonstrated knowledge and respect of, and ability to work within First Nations Traditional and Spiritual Institutes.

Hours of Work:
0830 to 1630 Monday to Friday (some flexible hours required)

Rate of Pay:
Salary commensurate with qualifications and experience.
