Diabetes & Wellness

The Diabetes & Wellness team provides education and clinical chronic disease management with a focus on diabetes. The team develops and delivers health promotion and prevention activities to promote and support overall good health in all ages and stages of life. The Diabetes & Wellness team also provides nutrition support to acute care at All Nations’ Healing Hospital (ie: tube feeds, dysphagia) as well as nutrition and diabetes care for dialysis patients. Our goal is to support clients in achieving their health care goals in a culturally safe environment. 

Wellness Clinics

Our team hosts Wellness Clinics in community health centres on a regualr basis to provide individual diabetes and nutrition counselling. We work alongside the home care nurses and community health nurses to provide access to footcare, immunizations, health education, and our fresh market initiative.

Anyone can attend! Contact your local health centre to find out when the next wellness clinic is scheduled.

About the Diabetes and Wellness Team

The Diabetes and Wellness Team includes Registered Dietitians, Certified Diabetes Educators, and Diabetes Nurse Educators.

Self-referral – contact a team member or FHQ health services to get connected with an educator in your community
