Home and Community Care

What is Home Care?

Home care is a program consisting of registered nurses (RNs), licensed practical nurses (LPNs), and home health aides (HHAs). Home care provides services to people of any age living in one of the seven communities we serve. We help people help themselves and assist people to stay in their own home with their loved ones for as long as possible. We work with other service providers to provide holistic care. For example, the Diabetes & Wellness Team, Rehabilitation Therapies, primary care providers, cultural support etc.

Our Services and Scope of Work

We provide culturally safe, client-centred care and supportive services.

  • Nursing assessment – Registered nurse reviews health history and helps make a plan for care
  • Nursing care in the home or community
  • Teach clients and family to help with self care
  • Monitor medications
  • Specialized wound care
  • Foot care
  • Order continence supplies
  • Order mobility equipment and aids
  • Speak on your behalf and make referrals
  • Assist with navigating the health care system
  • Follow up care from hospital to home
  • Palliative care
  • End of life care
  • Personal care (bathing) in the home, health centre, or at All Nations’ Healing Hospital
  • Home visits for elderly or those who live alone
  • Wellness Clinics at the Health Centre
  • Assessments for long term care
  • Health education

Communities We Serve

The Home and Community Care program is operated out of FHQ Health Services, located at All Nations’ Healing Hospital.

We offer services to seven First Nations communities within FHQTC:

How to Receive Home Care Services

  • Self-referral
  • Visit your primary care provider to request a referral
  • Speak to a member of your primary health care team
  • Contact the health centre in the community you live in
